a hart hub-funded initiative
The Bridge Integrated Care Hub
Bridging Gaps in Health, Housing, and Social Support
The Bridge Integrated Care Hub is a low-barrier initiative led by a collaborative partnership of local health and social service organizations committed to supporting individuals experiencing homelessness and other vulnerable populations. The Hub offers essential services —including food assistance, primary health care, mental health and addiction counseling, and vital connections to housing and social supports.
Homelessness casts a shadow over our community, intersecting with social, economic, and health crises, including mental health issues, addiction, income insecurity, and the lack of affordable housing. It strains public resources, heightens safety concerns, and creates a sense of disconnection for all residents.
Those without homes endure immense hardship, facing instability and the loss of basic needs. The causes and impacts of homelessness are complex, requiring a collective, compassionate effort to address this urgent issue.
On April 1, 2025, the services provided at The Bridge relocated to 125 South Church Street in Belleville, where they will remain until the renovations at the permanent home located at 1 Alhambra Square are complete. This is currently projected for late 2025 or early 2026.
The relocation of The Bridge services is more than just a move—it marks the first step in creating a more unified and coordinated care system for individuals living in the Hastings and Prince Edward region with complex needs. This integrated care model that will be implemented is part of the HART Hub, announced in January 2025.
